This book cover has stayed with me since seeing it. It's quite upsetting. Also treating it full bleed; they seem to have understood the impact of the photograph. Saw Romek Marber talk a couple of years ago — not sure if he was responsible for this one or for this mutation of his grid — but it was interesting how quickly, within limited means, that they had to work, especially in sourcing or shooting images. Somehow it gives credence to the first idea as the one you have to go with, however heavy-handed.
